Licensed Vocational Nurse (LVN)

Part-time LVN responsible for providing home health services consistent with the mission, vision and goals of Life Choice Home Health Care. Life Choice Home Health believes that health care is a basic human right. It must be available , coordinated, and provided in a comprehensive and affordable way that is combined with other human services when appropriate. Our team of home care professionals work with Physicians and the family to develop individualized, comprehensive, and cost-effective treatment plans. A Licensed Vocational Nurse (LVN) is responsible for providing direct skilled client care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes.

REQUIREMENTS
- Valid LVN license in the state of Utah
- Previous experience (1 to 2 years) working at a home
health agency
- Current driver's license and insurance
- Current CPR certification
- Demonstrated ability to execute an effective, helpful,
sensitive attitude towards patient/family
- Demonstrated ability to be committed, flexible, and
self-directed
