Seeking Nursing Home Clinical Liaison to serve as the primary contact with health care providers including physicians, health care facilities, case managers, social workers, discharge planners, insurance case managers, attorneys and community groups.

·        The Clinical Liaison works internally with the administrator, medical director, nursing and therapy to develop programs beneficial to our residents and medical community.

·        The Clinical Liaison works closely with all outside referral sources and families as the on-site assessment and referral/admission liaison and develops solutions that meet the medical community needs to result in patient referrals.

·        The Clinical Liaison develops community awareness of our clinical services and facilities, using consultative selling techniques to develop new accounts and maintain existing accounts to assure retention of the referral source.
