RN Case Manager - Home Health

Wesley Homes is more than just a retirement community in Des Moines and Auburn.  We’re also a full service Medicare Certified Home Health agency with clients all over the Puget Sound area. 

Wesley Homes “At Home” (WHAH) is currently seeking (2) full-time (32+ hours per week) RN Case Managers.  One Case Manager will be supporting our South King County and Pierce County areas and the other will be supporting our Seattle/West Seattle and North Seattle area.  Benefiits for these positions include Medical, Dental and Vision Insurance, 403b retirement savings account, Paid Time Off (PTO), and much more.

These positions provide direct care and case management for a team of WHAH patients in accordance with the state Nurse Practice Act and WHAH policy. These positions direct assigned team members of RNs, LPNs, and Home Health Aides and coordinates care with full range of therapists.  Our ideal candidates would be self-starters with strong nursing assessment skills, have the ability to formulate and complete a plan of care for patients living at home, and be able to communicate the needs of the patient to physicians involved with the patients care.  In addition, you must be able to manage a patient count of 15 - 25 patients during a 5-day work week.

HIRING REQUIREMENTS & PREFERENCES: 

  • Graduate of an accredited school of professional nursing
  • Current WA State license as a Registered Nurse (RN), BSN preferred.
  • Minimum of three (3) years of nursing experience, Home health experience preferred
  • Ability to supervise and direct members of patient care team
  • Demonstrated strong and effective written and verbal communication skills and good interpersonal skills
  •  Ability to work closely with patients, families, discharge planners, doctors and the community
  • Demonstrated organizational and time management skills
  • Knowledge of WA. State home health regulatory and reimbursement requirements
  •  A current and valid Washington State driver’s license and valid vehicle insurance
  • Ability to drive long distances in all weather conditions
  • Current CPR certificate
  • Ability to provide 3 professional reference checks
  • Able to clear background inquiries by DSHS and WSP
  • Able to pass drug screen pre-employment and random throughout employment
  • Able to provide documentation proving eligibility to work in the United States
