Occupational Therapist - Homecare
Make a Positive Difference
·Assesses each patient's environment and recommends adaptive equipment and/or home modifications as needed
·Helps patient achieve independence at home through activities of daily living training and functional activity training.
· Trains patients and/or caregivers in aspects of care that will be managed independently after discharge
·Plans adequately for discharge including referrals to appropriate community resources
·Communicates regularly with the patient's physician, home health care team, and other key members of the care team
·Bachelor’s degree in occupational therapy
·Currently licensed as an Occupational Therapist in the State of Maryland, Virginia or the District of Columbia
·One-year clinical experience
·Good verbal and written communication
·Good clinical skills
·Able to transfer, position patients and lift up to 25 lbs; able to perform physical assessments, walk and climb stairs
·Must have reliable transportation to all work sites
·CPR certification preferred
